Recommended Action:
To recover from this fault, try the following actions

1. Check if the FC Ports are configured in groups of 4. If not Correct it.
2. Check if the Ports are configured in Mixed Mode (Ex : 1-4 Ethernet 5-8 FC 9-12 Ethernet). If yes Consolidate them in Two Groups.
